Forms and templates:

Learning Agreement - The template for your ERASMUS+ Learning Agreement is provided in your participant's account once you have been given the status perticipant by the administrative staff of the IO. This is only possible after your departmental coordinator has completed the internal nomination by April 1st. Here you find a trial copy.

Grant Agreement - a contract between participant and home university detailing the different aspects of the ERASMUS student mobility incl. funding. It is provided in your participant's account once the learning agreement has been checked as correct there. Please read and sign your grant agreement and upload it into your account. Here you find a trial copy.

Confirmation Period of Study (pdf) (Arrival) - is provided in your participant's account once the grant agreement has been ckecked as correct there. Please get it confirmed (signed AND stamped) at the international office of your host institution or with the departmental coordinator there.

Changes to Original Learning Agreement - If changes to your original learning agreement are necessary once you have arrived at the host institution, you find the changes to learning agreement-template in your participants account. This is possible only after the arrival certificate has been checked as correct.

Confirmation Period of Study (Departure) - was provided in your participant's account at the start of the mobility and can be downloaded there again. Please go to the international office of your host institution an get the confirmation of the end date of your ERASMUS study period. Once it has been signed and stamped please upload it into your participant's account. 

EU Online Survey - is provided by the EU Commission through the EU Mobility Tool. You will receive an email with the link to the online survey approximately 2 weeks after the planned end date of your study period. Unfortunately it is not possible for the international office of Goethe-University to send you this link or speed this process up. 

Personal written report/testimonial (doc-template) - The template will also be provided in your participant's account once the confirmation of period of study has been checked as correct. You have to write a minimum 3-page report about your ERASMUS experiences and upload it into you participants account.

Final Grant Agreement - an updated version of the original grant agreement. It will be provided in your participants account once the confirmation of period of study has been checked and the start and end date have been updated and the grant sum has been recalculated. Please read and sign your final grant agreement and upload it into your account.

Course Certificate for Temporary Students (pdf) (Template of a course certificate to confirm the participation and/or grades of single ERASMUS courses. This document is not a mandatory document, but might help in certain situations and with the recognition of ERASMUS credits upon return to Goethe-University.)
